I would see that doctor sooner rather than later. Your boyfriend might be right on the disease, just not the type. While you are small and also young to have type 2 diabetes, you might have type 1 (which is younger age onset). Type 2 is usually diagnosed in older people and is caused by the bodies cells losing the ability to use the insulin produced in the pancreas. Type 1 on the other hand is a result of the pancrease not producing the insulin in a great enough quantity.
Your symptoms of being very thirsty and having to go to the bathroom alot are two of the common ones for diabetes. The leg twitches are probably realated to the dehydration (causing electrolyte imbalances).

You do NOT have to be overweight to have diabetes. If you have been hospitalized, I would assume you were tested for diabetes, but, you know what happens when you assume!
If we use the diagnostic principle of Occam’s Razor (the principle suggests that looking for the underlying problem/condition that is causing all of the patient’s symptoms is the best approach to finding the answer) and ask what do diabetes, migraines, kidney problems and muscle spasms have in common? The answer is Magnesium deficiency.
It might be productive to have a thorough workup for magnesium status (NOT just serum magnesium levels) and renal handling of magnesium.

Have you had your blood sugar levels monitored? All of those things are signs of diabetes. And yes, you can develop it even if you aren’t overweight. I’m “borderline” and I’m 29, 120 lbs, a size 3, and I’ve never been overweight. I get headaches, experience frequent thirst (not as bad as you, though), have to pee more than seems necessary, and I have a TERRIBLE craving for sugary things most of the time. Yet eating sugar often makes me thirsty again.
Check out the diabetes thing first, and if your blood sugar levels are normal, try an endocrinologist. You may have a hormonal disorder or a thyroid problem.
Edit: Magnesium deficiency is also a good suggestion. That can also cause headaches.
