A Poor Functioning Gluteus Maximus Could Be the Cause of Your Low Back Injury

A common neuromuscular dysfunction found in clients is poor muscle firing of gluteus maximus. Gluteus maximus is a large muscle and very obvious in most clients but its size and prevalence does not mean it is working properly.

A common response when you tell someone that they have poor firing gluteus maximus is they let you know that they do a lot of running, walking, squats and lunges. All of these exercises are great at targeting the gluteus if they are done right but if the gluteus muscle is not activating properly, the hip extensors (most commonly the hamstrings) end up doing all the work.

With more and more people involved in desk work involving sitting, complaints about the gluteus maximus are more common. In a sitting position, the hip joint moves into flexion. This leads to an altered length-tension relationship between muscles, a subsequent shortening of the hip flexors (iliacus, psoas, tensor fasciae latae, rectus femoris, sartorius, adductor longus, adductor brevis, pectineus), and lengthening of gluteus maxiums and adductor magnus. Along with hip flexion, the knee is put into flexion leading to an altered length-tension relationship of biceps femoris, semitendinosus, semimembranosus, gracilis, sartorius, popliteus and gastrocnemius into a shortened position.

Alternation of the normal length tension relationship takes the body out of the optimal neuromuscular control and into dysfunction. This leads to a common pattern of overactive / tight and underactive / weak muscles around the lumbo-pelvic-hip complex (LPHC).

The muscles that are commonly found to be overactive / tight around the LPHC are: iliopsoas, erector spinae, piriformis, tensor fascia latae, adductor complex and latissimus dorsi.

The muscles that are commonly found to be underactive / weak around the LPHC are: gluteus maximus, gluteus medias and gluteus minimus.

This pattern of active and under active muscles is often seen in a lower crossed syndrome client. A key feature of this posture is an anterior pelvic tilt.
